Job Description

Job Title: Clinical Pharmacist
Location:
Saranac Lake, New York

Schedule: Full-Time | 40 Hours per Week

Compensation: $65.00 per hour ($135,000 annually)

Job Overview

A well-established healthcare organization in the Adirondack region is seeking an experienced Clinical Pharmacist to join its Pharmacy Services team. This role is ideal for a pharmacist with a strong clinical background who is passionate about optimizing medication therapy, collaborating with healthcare providers, and delivering exceptional patient care in a hospital setting.

The Clinical Pharmacist will be responsible for the safe and effective dispensing of medications and pharmaceutical supplies while providing clinical support to physicians, nursing staff, and patients.

Responsibilities

  • Review and verify medication orders for accuracy, appropriateness, and safety
  • Dispense medications and pharmaceutical supplies in accordance with department policies and regulatory requirements
  • Provide clinical pharmacy support and medication therapy recommendations
  • Monitor patient medication regimens for effectiveness, interactions, and adverse effects
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
  • Counsel patients and families regarding medication use when appropriate
  • Maintain accurate pharmacy records and documentation
  • Ensure compliance with state, federal, and hospital regulations
  • Participate in quality improvement and patient safety initiatives

Qualifications

Required

  • Current New York State Pharmacist License
  • Minimum of 2–3 years of clinical pharmacy experience
  • Previous hospital pharmacy experience
  • Strong understanding of medication management and clinical pharmacy practices
  • Excellent communication and collaboration skills

Preferred

  • Experience working in an acute care or inpatient hospital setting
  • Strong clinical decision-making and patient care skills
